Preparing For Your Appointment

Pre-Consultation Questionnaires

Our doctors would like to be able to cater for your needs more promptly, hence we ask that parents or carers kindly assist us by completing a set of Pre-Consultation Questionnaires.

What To Bring For Your Appointment

If you are concerned about your child’s development, learning or behaviours, AND your child has not had an updated hearing assessment (Audiology testing) in the last 2 years, please kindly organise one prior to seeing us. You may obtain a referral from your GP.

If you are concerned about your child’s development, learning, or behaviours, AND your child has not had an updated vision test in the last 12 months, please kindly organise one prior to seeing us. You may source a Paediatric-aligned optometrist in your local area here.

If your child has had blood tests or other pathology tests done relevant to the current presentation, please kindly request your GP to forward the test results to us.

Please kindly forward any relevant past reports from your allied health clinicians to us prior to your appointment.

Please kindly forward any relevant reports from your child’s education institution as you see fit.

Please kindly forward any relevant past medical records as you see fit.

Please kindly speak to your child about the reason they are coming to the appointments prior to your visit. As the appointments (especially the first visit) can sometimes be very lengthy, please ensure your child has something to eat prior to the appointment, as well as to prepare some preferred activities to occupy them while waiting.

Nursing room and infant change table are available.

Yes, we do!

All telehealth and phone call appointments are regarded as standard appointments and will incur out-of-pocket charges, regardless of whether Medicare rebates are applicable.

Medicare offers rebates for telehealth / phone appointments if your child is present during the consultations.

Our practice has a strict cancellation policy. Please kindly add an appointment reminder in your diary or calendar to remind yourself as soon as you book your appointment. Please ensure a referral is obtained prior to the appointment, if required.

Appointment confirmation and reminders are sent out via SMS to your nominated phone number, approximately 7-14 days prior to your appointment. Failure to respond to the SMS will result in your appointment being cancelled and offered to other children on the cancellation list.

If you would like to reschedule an appointment, please kindly phone our reception on (07) 3543 8888 or email admin@mypaeds.au

Late cancellation on the same day of your appointment will require a deposit of $100 to rebook the next appointment. This deposit will be credited towards your next appointment.

Failure to attend (No-Show) on confirmed appointments will incur a non-refundable fee of $100, before a rebooking can be made.

The practice reserves the right to not offer further appointments after two consecutive late cancellation or No-Show, due to the impacts these cancellations might have on the other children or families who are in need of our care and services.

Completion of forms (e.g. Centrelink, NDIS forms etc) and reports (e.g. NDIS reports, letter of support for school etc) require access to patient’s charts and the process takes time for our team to provide the services.

If you have forms to be completed outside the scheduled appointment times, please kindly book in a phone appointment to discuss these. Charges might be applicable.

All script requests must be booked into the Script Clinic slots due to the need to access patient’s charts and carefully confirming details such as medication’s name, dose and supply required.

Please note that our admin team is not allowed to take email or verbal request over the phone for medication scripts.

Your doctors will have allocated time slots for telehealth / phone consultations specific for script requests.

Script appointments will be charged at $25 if your child is not present on the phone call.

If your child is present during the phone call, the appointments will be eligible for the Medicare rebate.

If yourself or your child developed infective symptoms (respiratory/gastro), please do not attend the appointment.

There are options to convert your scheduled face-to-face appointment into a telehealth session, if you feel fit for it. Alternatively, please give us a call or email at your earliest convenience to notify us and discuss rescheduling and securing the next available appointment.

The parent initiating care should provide MyPaeds Specialist Clinic and the treating team, the up-to-date care arrangement documentation (eg. parenting agreement or court order), if applicable. The other parent / guardian should be offered to participate in the medical care of your child including attendance to appointments, participation in assessment process and completing questionnaires requested by treating team.

The treating team and MyPaeds Specialist Clinic hold no responsibility in keeping the other parent informed about the ongoing medical care, including decision making for assessment and treatment plan including the use of medication and therapy intervention.

Both parents generally have the right to access health information about their child (whether they are divorced or separated) unless a court order places restriction on a particular parent from accessing the information.

In instances where one of the parents cannot be present (either in person / via phone or video call) for consultations requiring certain decision making by BOTH parties, another appointment will need to be made with additional standard consultation private fee payable, and NO further Medicare rebates applicable.

Please seek appropriate legal advice if there are disputes between parents / guardians, or you are unsure whether you can provide consent or access health information of your child.
